Caring Hands Mobile Phlebotomy (CHMP),LLC is responsible for drawing and delivering specimens to the lab for processing. CHMP does not collect ANY type of specimen without the order from a Physician or a lab labeled requisition form. We do not give any medical advice, treatment or diagnose any patient.
Caring Hands Mobile Phlebotomy, LLC will handle all deliveries of samples to the lab. If a sample is being collected for a Laboratory that needs to be picked up via courier service, CHMP is not responsible for any delay of arrival to the laboratory once the sample has been delivered or picked up by the courier service.
Before arrival for any appointments, Caring Hands Mobile Phlebotomy will need proper information collected, i.e., patient ID, copy of insurance card (front and back) and lab orders. This information will need to be submitted to our office within 48 hours prior to appointment time to ensure accuracy for the staff. If information is not received within the allotted timeframe, the appointment will need to be rescheduled until all proper information is provided.
Caring Hands Mobile Phlebotomy is not responsible for any errors that may occur at the laboratory. It is the laboratory’s responsibility to process specimen once received by CHMP. In the event of a lab error, CHMP will gladly return to recollect the sample at a discounted rate. Any error made by CHMP will be corrected at no extra charge.
Any billing questions or problems for test collected for this service should be directed to your insurance company or the laboratory billing department.
Payment for services provided is collected the day before or the day of service.
CHMP is an independent convenience Service. Our company is not affiliated with any Laboratories, Doctor’s office or hospitals. CHMP does not have authorization to give discounts for any lab test that are being collected.
CHMP does not bill insurance companies, any claims must be submitted by the client, with no guarantee of reimbursement.
Caring Hands Mobile Phlebotomy, LLC will not provide personal information to any unauthorized individuals or entities, unless written authorization is provided by the patient to CHMP
All client information is always kept private and confidential.
Caring Hands Mobile does not receive results on behalf of our clients. Any results for any tests will need to be retrieved from your physician, who is the responsible party where the results are sent.
Cancellation Policy:
In the event that an appointment needs to be canceled, a 24-hour notice will need to be given to Caring Hands Mobile Phlebotomy so that the appropriate steps can be taken with the staff and another appointment can be made in a timely manner that is convenient for you and the company.
All cancellation will be charged an inconvenience fee of $25 to reschedule. If an appointments needs to be rescheduled, please send a request to rescheduled via email(info@caringhandsphlebo.) Please include your name, original appointment time and address in the email.
